To the Parents/Guardians of 5th Grade Students:

The State of New Jersey Department of Education has revised the administrative rules N.J.A.C. 8:57-4 to include the requirement of two vaccines for school attendance.
 
Listed below are the two vaccines that are required for those students entering the 6th grade for the 2022-2023 school year. A student born on or after January 1, 1997 who is entering 6th grade must have received both the Tdap and Meningococcal before the first day of the new school year.
 
1.   N.J.A.C. 8:57-4.10 - Diphtheria and tetanus toxoids and pertussis vaccine: Every child born on or after January 1, 1997 and entering or attending Grade Six on or after September 1, 2008, shall have received one dose of Tdap (Tetanus, diphtheria, acellular pertussis) given no earlier than the child’s 10th birthday. Documentation shall be provided as soon as it is administered.
 
2.   N.J.A.C. 8:57-4.20 - Meningococcal vaccine: Every child born on or after January 1, 1997 and before attending Grade Six on or after September 1, 2008, shall have received one dose of a meningococcal-containing vaccine given no earlier than the child’s 11th birthday. Within 2 weeks of the 11th birthdate, students must show documentation that they received the vaccine. Documentation shall be provided as soon as it is administered.
 
Students will be excluded from school if they do not have the required vaccines.
 
Please be advised that the nurse in your child’s building will be contacting you regarding the specific immunization requirements for your child. This letter is being provided at this time to give you the opportunity to discuss these requirements with your physician and make the necessary arrangements. If you have any questions, please call the school nurse at your child’s school.
