Dear Parents/Guardians:
 
KINDERGARTEN REGISTRATION for children who plan to attend the Eatontown Public Schools in September 2021 will be held as follows due to COVID-19 precautions:
 
DATES:
Monday, March 1, 2021 to Friday, March 26, 2021.
Information must be returned to the school by Wednesday, March 31, 2021.
 
TIMES:
Parents/Guardians may download and print a registration packet online at https://www.eatontown.org/o/eatontown/page/kindergarten-preschool-registration or pick up a registration packet at the school from 9:00 a.m. to 3:30 p.m.

REQUIREMENTS:   Children must be five (5) years of age before October 1, 2021. There are no exceptions.
 
You will need copies of your driver’s license or photo ID along with your child’s original, state-issued birth certificate, passport or adoption papers, proof of Eatontown residency (a current lease, tax bill, or utility bill), as well as the following medical records when you complete the registration packet:
 
REQUIRED VACCINATIONS:
 
DTP (Diphtheria, Tetanus, Pertussis): A minimum of 4 doses if one dose was given on or after the 4th birthday, OR any 5 doses
 
Polio: A minimum of 3 doses if one dose was given on or after the 4th birthday, OR any 4 doses
 
Hepatitis B: 3 doses
 
MMR (Measles, Mumps, Rubella): 2 doses
 
Varicella: One dose on or after the 1st birthday, or proof of disease immunity
 
Students not in compliance with immunization requirements will be placed on provisional status in September. To remain in school, the student must receive any overdue vaccinations by October 8th.
 
PHYSICAL: A physical examination by the child’s private physician (on or after September 8, 2020) is required before the student enters school in September. This is a state requirement. A copy of the Physical Form is included in the downloadable registration packet at the link above.
