Dear Parents/Guardians:

I have some exciting news. Preschool Registration for children who plan to attend Eatontown Public Schools in September 2023 has been extended due to the New Jersey Department of Education notifying the district in July 2023 that another preschool classroom has been funded by the state. Due to a limited number of spaces, please register your child as soon as possible to secure a seat for September 2023. Due to timing concerns, registration will be on a first-come, first-serve basis until the seats are filled.

Registration Packet Pick-Up

Parents/Guardians may pick up a registration packet from the Administrative Offices between the hours of 9:00 a.m. to 12:00 p.m. on Monday – Friday.

PLEASE NOTE: Preschool registrations must be completed and returned to the Administrative Offices in order to secure a seat in the Preschool program.

REQUIREMENTS

Preschool: Children must be three (3) years of age on or before October 1, 2023.

There are no exceptions.

You will need copies of your driver’s license or photo ID along with your child’s original, state-issued birth certificate, passport or adoption papers, proof of Eatontown residency (a current lease, mortgage statement, or tax bill), as well as the following medical records when you complete the registration packet:

Required Vaccinations for Preschool Students:

  1. DTaP (Diphtheria, Tetanus, and acellular Pertussis): A minimum of 4 doses

  2. Hepatitis B: 3 doses

  3. Haemophilus influenza type b (Hib): A minimum of 4 doses if one dose was given on or after the 1st birthday

  4. MMR (Measles, Mumps, Rubella): 1 dose

  5. Pneumococcal conjugate (PCV 13): A minimum of 4 doses if one dose was given on or after the 1st birthday

  6. Inactivated Poliovirus (Polio): A minimum of 3 doses

  7. Varicella: One dose on or after the 1st birthday, or proof of disease immunity

  8. Influenza (IIV; LAIV): One dose due each year between September 1st and December 31st.

Students not in compliance with immunization requirements will be placed on provisional status in September. To remain in school, the student must receive any overdue vaccinations by October 8th.

PHYSICAL: A physical examination by the child’s private physician (on or after September 7, 2022) is required before the student enters school in September. This is a state requirement. A copy of the Physical Form will be provided as part of the registration process.
