Documentation shall be provided as soon as it is administered.
  1. N.J.A.C. 8:57-4.20        Meningococcal vaccine: Every child born on or after January 1, 1997 and before attending Grade Six on or after September 1, 2008, shall have received one dose of a meningococcal-containing vaccine given no earlier than the child’s 11th birthday. Within 2 weeks of the 11th birthdate, students must show documentation that they received the vaccine. Documentation shall be provided as soon as it is administered.

Students will be excluded from school if they do not have the required vaccines.

Please be advised that the nurse in your child’s building will be contacting you regarding the specific immunization requirements for your child. This letter is being provided at this time to give you the opportunity to discuss these requirements with your physician and make the necessary arrangements. If you have any questions, please call the school nurse at your child’s school.

Sincerely,
